When registering in our secure, HIPAA compliant patient portal site, we request your personal demographics (name, DOB, etc.) and insurance card information, social security number, and email address. The email address you provide may be used to send information, and respond to questions.This information is used to provide us with the information needed to verify your insurance benefits and to create your medical record within our practice. All information is held secure within the system.Your information, whether public or private, will not be sold, exchanged, transferred, or given to any other company for any reason whatsoever, without your consent, other than for the express purpose of delivering the purchased product or service requested.
